Case Logs - REQUIRED Template

Excel version of the ABVLM Case Log. Please complete the log(s), save a copy, and upload it with your application. NOTE: you must use this version of the log.

NOTE: If you are documenting 100 ultrasound cases, you DO NOT need to fill out a separate log. You can use the experience case log and fill in the diagnostic ultrasound column as it correlates to each case/patient. If there are multiple cases for a single patient, you can only count the diagnostic ultrasound once; thus, you will need to add additional ultrasound cases to make 100. However, you can also submit two log files.

Starting with 2013 exam cycle, the Retake Process is now also fully online. If you cannot login to the Online Application screens and you wish to re-take the exam from any former year, please contact the HQ Office.
