How should I fill out the case log to make sure that I have listed 200 vein treatment cases and 100 Ultrasound diagnostic experiences?
This requirement does not mean that you need to submit two separate case logs. Some of your vein case experience will likely include ultrasound (US) diagnostic experience. Identify 200 cases that document your venous & lymphatic clinical case experience. Review each case as you list them, and if they include US diagnostic experience, you can indicate that on the same line. Once you've completed this listing, check how many include US. If there are less than a 100 (which include US), then you have to list more cases documenting US experience.
